*SUMMARY:* Provides services to Veterans at the Emergency Housing Program.
This program provides temporary emergency shelter for up to 90 days (may be extended by authorization of VA personnel only) as required by the Department of Veterans Affairs (VA).
Assists Veterans with activities of daily living, meals, and housekeeping.
Provides and links with services including therapeutic, rehabilitative and recovery services, structured group activities, VA coordination, clinical case management in collaboration with VA, living skills, housing search, outreach, and discharge planning.
Assists Veterans with progressing on their care plan goals.
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential job functions.
Caregivers working on the NOC shift are allowed to sleep during the shift.
Other duties assigned as needed.
*ESSENTIAL DUTIES AND RESPONSIBILITIES:* Must be able to perform each essential job function satisfactorily.
*Cooking and Housekeeping* * Follow shift specific guidelines to ensure that three meals per day plus nutritious snacks are provided on time according to House Policies.
* Abide by the principles of good nutrition, food preparation, storage, menu planning, cooking, clean-up, food safety and food service principles.
* Follow provided menu and recipes, adhering to special diets as prescribed by physicians.
* Be knowledgeable on housekeeping and sanitation principles.
Perform housekeeping duties as necessary.
* Assist and supervise Veteran daily chores including cleaning after meals.
Ensure that Veteran chores are completed abiding by the principles of housekeeping and sanitation principles.
Ensure that each Veteran completes his assigned chores on a daily basis.
* Assist Veterans in maintaining cleanliness of living areas, including Veterans’ rooms and community areas.
Ensure that Veterans perform their own laundry.
*Veteran Services* * Assist in identifying and accessing medical, educational, social, prevocational, rehabilitative, or other community and VA services for Veterans, specific to Veterans’ needs, goals, care plans and housing.
* Assist Veterans with daily living activities including cooking, cleaning, showering, hygiene, and grooming, and other personal care assistance.
Provide Veterans with counseling, training, and assistance with daily living skills.
* Assist Veterans as necessary in attending appointments, outings, etc.
* Monitor and report changes in Veteran’s physical or mental health status to the Veteran’s primary care providers and House Manager.
* Initiate preventative measures and create intervention methods for Veterans or conditions that pose a threat of danger or crisis.
* Report any substance use by Veterans to House Manager.
* Inform Veterans of personal rights and responsibilities, as well as agency confidentiality and grievance procedures.
* Maintain cooperative and professional working relationships and communications with Veterans, co-workers, family members and community partners.
* In a culturally competent manner, provide services to all Veterans without regard to race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, handicap, and all other protected groups.
* Provide input to management on improving the quality of programs and assist in monitoring program effectiveness.
* Other duties and responsibilities as assigned.

* Help organize and conduct planned activities.
Document mandatory group attendance by Veterans and notify supervisor of non-compliance.
*Reporting and Documentation* Report any abusive or disruptive behavior to the House Manager and team.
* Ensures the privacy and confidentiality of Veteran’s health information is protected in compliance with contractual requirements and local, state, and federal laws including HIPAA and Welfare and Institutions Codes.
* Responsible and mandated to report any “suspected abuse of dependent adults and elders” directly to law enforcement and Adult Protective Services and any other agency as appropriate.
Mandated to comply with suspected abuse reporting policies and procedures.
* Keep clear, accurate, and professional records in Veteran’s files, staff logs, and other required communications in compliance with QASP and SOW requirements.
* Meet FSI guidelines for documentation.
* Communicate (written, verbal, and in-person) in a professional manner.
*QUALIFICATION REQUIREMENTS:* Knowledge, skill, or ability required to perform the job.
*Education and/or Experience* * High School diploma or GED; one year relevant experience may be required; or equivalent combination of education and experience.

*Mathematical Skills* * Calculate figures and amounts such as discounts, proportions, and percentages associated with completing the requirements of the position.
*Reasoning Ability* * Ability to carry out instructions furnished in written, oral, or diagram form.
* Competent in dealing with problems involving several variables in different situations.
*Other Skills and Abilities* * Adjust tasks in accordance with changing deadlines and priorities.
* Maintain cooperative and professional working relationships with others.
*Additional Requirements* * Must be 18 years of age or older.
* If required to drive, valid California Driver’s License and corporate vehicle insurance eligibility and approval.

* Cleared by criminal background check prior to commencing work at the facility.
* Maintain valid First-Aid and CPR certifications.
* May be required to serve on company committees.
* Able to answer telephone professionally, understandably, and with proper etiquette.
* Attend mandated meetings and trainings; fulfill all coursework and training hours required.
* Work in a safe and acceptable manner, following established safety procedures.
* Resume subject to approval by VA liaison prior to hire.

* Regularly required to talk, hear, sit, and stand for extended periods of time.
