This position will be located in Santa Cruz county.
Our facilities specialize in providing services to adults with mental health disabilities while promoting healthy living and wellness in a spectacular setting.

SUMMARY: Provide care and supervision to residents as required by Title 22, Community Care Licensing Division.
Assist residents with activities of daily living.
May prepare meals and provide housekeeping for residents as needed.
Implement and assist residents to comply with their individual Appraisal/Needs and Services Plans.
Perform overall management of medication room, its systems and supplies, and enforces adherence to FSI medication policies, procedures and protocols.

ESSENTIAL DUTIES AND RESPONSIBILITIES Must be able to perform each essential job function satisfactorily.
Resident Care In a culturally competent manner, will provide services to all residents without regard to race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, and handicap or payment status.
Assist residents with daily living activities including cooking, cleaning, showering, hygiene and grooming, and other personal care assistance.
Provide necessary resident care and supervision as prescribed by doctors, other health care providers, and as developed by the resident’s treatment team.
Safely assist residents with self-administration of prescription and non-prescription medications in accordance with physicians’ orders and Title 22.
Assist with planning and implementing individuals’ treatment plans in conjunction with other staff, family and providers.
Monitor and report changes in residents’ physical and mental health status to the resident’s primary care providers and facility staff.
Assist in identifying and accessing medical, educational, social, prevocational, rehabilitative or other community services for residents.
Monitor and report residents’ progress and outcome data.
Track and schedule residents’ annual dental and physician’s exams including TB tests.
Transport residents to appointments, outings, etc.
Other duties and responsibilities as assigned.
Cooking Abide by the principles of good nutrition, food preparation, storage, menu planning, cooking, clean-up, food safety and follow Title 22 and food service principles.
Conduct menu planning with residents, adhering to special diets as physician prescribed.
Cleaning Be knowledgeable on housekeeping and sanitation principles.
Perform housekeeping duties.
Maintain living areas, including resident rooms and community areas.
Activities (As Designated) Help organize and conduct planned activities.
Gives capable residents the opportunity to participate in the planning, preparation, conduct, clean-up and critique of the activities.
Adheres to monthly schedule of planned activities.
Reporting and Documentation Mandated to report any suspected abuse of dependent adults and elders directly to Law Enforcement, Adult Protective Services, Ombudsman, or Community Care Licensing.
QUALIFICATION REQUIREMENTS: Knowledge, skill, or ability required to perform the job.
Education and/or Experience High School diploma or GED; one year mental health experience may be required; or equivalent combination of education and experience.
Knowledge Knowledge of requirements for providing care and supervision of residents.
Knowledge and conformity to all applicable HIPAA, OSHA, state and local laws, rules and regulations including Department of Social Services, Community Care Licensing, Title 22.
Knowledge of community services and resources and how to access them.
Language Skills Read, speak write and understand English well enough to follow and share the regulations, requirements and communications for providing care to residents.
Able to effectively present information and respond to questions from managers, residents, customers, vendors and the general public with awareness of multi-cultural considerations.
Mathematical Skills Calculate figures and amounts such as discounts, proportions, and percentages associated with completing the requirements of the position.
Reasoning Ability Ability to carry out instructions furnished in written, oral, or diagram form.
Competent in dealing with problems involving several variables in different situations.
Other Skills and Abilities Adjust tasks in accordance with changing deadlines and priorities.
Maintain cooperative and professional working relationships.
Additional Requirements Must be at least 18 years of age.
If required to drive, must be 21 and accepted by the Company’s vehicle insurance.
Cleared by the Department of Justice and FBI prior to commencing work at the facility.
TB clearance within the last 6 months and yearly thereafter.
Pass a health screening.
Maintain a valid First-Aid certification.
May be required to serve on company committees.
Able to answer telephone professionally, understandably, and with proper etiquette.
Complete all annual training requirements as mandated by Community Care Licensing Division, Title 22 and Front St.
policies.
Attend mandated meetings and trainings; fulfill all coursework and training hours required.
Complete coursework, training hours and pass the Medication Policies and Procedure Examination.
Valid water safety certificate may be required for facilities with a resident-used pool.
Work in a safe and acceptable manner, following established safety procedures.
Physical Demands: Must be met to successfully perform the essential job functions.
Regularly required to talk, hear, and sit, stand for extended periods of time.
Frequently required to cook and clean, reach with hands and arms, stand, walk, stoop, lift up to 25 pounds, and may occasionally be required to kneel, crouch or crawl.
Be physically, mentally and occupationally capable of performing assigned tasks per Title 22 and other applicable regulation.
Work Environments: Employee may encounter while performing essential job functions.
Noise level is usually low to moderate.
Possible contact with hazardous and caustic household cleaning chemicals.
The actions of clients may be unpredictable.
Staff may be verbally assaulted and at times may be physically injured.
